At Prestige Medical Group, we are committed to providing you with the highest level of care, even when you aren't in our office. Our Chronic Care Management (CCM) program is designed specifically for patients with two or more chronic conditions—such as diabetes, CHF, COPD, or CKD—that are expected to last at least 12 months.

Our goal is to help you manage your health proactively, reduce the risk of hospitalization, and ensure you have a personalized roadmap to wellness.

What is Chronic Care Management?

CCM is a specialized service that provides you with a dedicated care team to oversee your health needs every month. This program includes:

• Personalized Care Planning: We create and monitor a care plan tailored specifically to your health goals.
• Medication Management: Ongoing review and reconciliation of your medications to ensure safety and effectiveness.
• Provider Coordination: We act as your central hub, coordinating care with your specialists, hospitals, home health services, and other providers.
• 24/7 Access: You gain 24/7 access to care management services for peace of mind.
• Monthly Touchpoints: At least 20 minutes per month of dedicated, non-face-to-face care management to keep you on track.

Important Program Details

• Cost-Sharing: Medicare applies the usual Part B cost-sharing (deductibles and copays may apply).
• Exclusivity: Only one healthcare provider can bill Medicare for CCM services in a given month.
• Flexibility: You are in control. You may stop these services at any time, effective at the end of the calendar month.
• Comprehensive Care: These services are provided outside of your regular office visits to ensure continuous support.
